HEAD-SHOT PHOTO INSTRUCTIONS: (see field below)
1. Ensure that the entire front of the child's face is captured in the photo and that it is the main subject.
2. Ensure that the photo is a close-up of your child and minimize the background information in the photo.
3. Landscape orientation is preferred over a portrait orientation.
4. Ensure that there are no other people included in the photo.
5. Ensure that the photo is recent. It is even preferred that the uploaded photo be taken at the time of this registration.
6. Smiles are best. Silly faces are not appropriate for this photo upload.

This email will include an online link to the Calvary Road Baptist Church Medical Release Form that is required for every child who is registered in our AWANA Clubs program. Parental Consent Checklist
Please read through the below statements of parental consent. By clicking the Submit button at the bottom of this form, you are indicating that you will abide by each and every statement for as long as your child is enrolled in the Awana Clubs ministry of Calvary Road Baptist Church.
1. I understand that my child (identified in this form) will participate in physical activities such as those held during Game Time. As with any physical activity, there is a risk of injury. I fully accept this risk and hold harmless from any/all legal liability, Calvary Road Baptist Church, and any/all person(s) involved with the Awana Clubs Ministry of Calvary Road Baptist Church.
2. In the event of a medical emergency that requires medical treatment for my child (identified in this form), I undestand that every effort will be made to contact me or my designated Emergency Contact prior to any medical treatment. However, if parent/guardian and Emergency Contact person cannot be reached, I give my permission to the Awana Clubs volunteers/Church Staff to secure the services of a licensed medical professional to provide the care necessary for my child's well-being. I assume responsibility for all costs connected to any accident and/or treatment of my child.
3. I grant permission for a photo of my child (identified in this form) to appear in an unpublished club directory to be used by the Awana Clubs leadership. I also give permission for photo(s) of my child (identified in this form) to appear among other general club photos as long as there is no identifying information shown in the photo.
4. I grant permission for my child (identified in this form) to travel to/from Awana Clubs events with, at least, two authorized/approved adult Awana Leaders in an approved church-owned and/or church-approved passenger vehicle. NOTE: Any such travel event will be clearly communicated with parent beforehand.
